4. Health Behavior Change Journal- this is an Individual Assignment: (10 points)

For approximately one week, students will have the opportunity to attempt to make a health behavior change in a health area of their choice. This can be an area you want to maintain, improve, increase/decrease depending on the type of change. Examples: eat better/healthier, exercise (start or increase), manage stress, improve sleep, quit/decrease smoking, and/or improve time management. For approximately one week, students are being asked to keep track (on computer, on a notebook paper) of your actions taken during this time period. There is no right, wrong, and no failure. Research shows it takes 3-4 months of consistent practice to make a behavior change stay. The mere act of attempting a health behavior change makes you a success!

Time Frame: 1 week (approximately 5 to 7 days): Start date-anytime from January 3rd.

Due Date of your completed journal AND responses to questions #1-6 below: Saturday, January 12th on/by 11 pm. UPLOAD your assignment in tab “Health Behavior Journal”

Please follow this format and include in your final journal paper. You should have at least ONE entry per day for 1 week for approximately a total of 5 up to 7 entries.

7. After you have attempted your behavior change for 5 to 7 days, SUMMARIZE, in approximately 1 to 2 pages maximum how you did. Speak to the challenges, your attitude towards this change, and if you will try to continue with your behavior change.

• Journal should have at least ONE entry per day. An entry can be a paragraph, a page, or several pages depending on how YOU manage your behavior change. The goal is to show you are attempting, each day, to work on improving your overall health/life. Please use an organized format, make it clear to decipher your responses to questions #1-6 and #7 Summarize 1 page, and include your actual entry dates/feelings/thoughts/emotions around the behavior change.
