How does the information in the article apply to the project problem or proposed intervention?

PICO Worksheet What is the PICO question? Does the use of a standardized bedside report versus board reporting help increase patient safety, nurse’s satisfaction and positive outcome? Define each element of the question below: P- (Patient, population, or problem): patients and nurses I- (Intervention): bedside reporting C- (Comparison with other treatment/current practice): bedside reporting O- (Desired outcome): Increase in patient’s safety, satisfaction and outcome. What is the practice issue/problem? Bedside reporting has been an issue in practice to this day. It serves as open communication between nurses at shift change, allowing nurses to exchange necessary patient information to ensure continuity of care and patient safety. It also allows the patient the ability to contribute to his or her plan of care. Most importantly, it allows the oncoming nurse an opportunity to meet, assess the patient and ask questions. Unfortunately, not all nurses are being compliant with this practice for that reason I chose this topic to help and encourage nurses the importance of bedside reporting.

What is the practice area? _x__ Clinical ___ Education

Evidence Appraisal Worksheet Your Name: Razelflor Kelsen Date: Jan 12, 2017 Your Instructor’s Name: Prof. Candice Phillips Purpose: To find evidence to support an intervention that will change the outcomes. Directions: Type your search question below. Find AT LEAST FOUR sources to support the need for change and the potential intervention you have selected to solve the problem. At least three of your sources must be peer-reviewed articles. The fourth source could be another peer-reviewed article or a reliable, credible source. Look in your text for ideas of other sources that can be used. Using the table below, insert and describe your four chosen resources. Step 1: APA Reference for the article. You will need to list the reference for the source in APA format. Be careful when using built-in APA formats and library citations. They may not be in APA format. Refer to Chapter 7 of your APA manual.

“Value of Bedside Shift Reporting” is a study to explore nurse’s experiences and perceptions associated with implementation of bedside nurse-to-nurse shift handoff reporting. The authors used interviews to conduct this qualitative study that was undertaken in an inner city, acute care teaching hospital. The sample was adequate for the design that consist of female participants from different clinical cohorts: general surgery (16), nephrology (10), OB and Respirology (7) and average working as a nurse was 0.5-41 years. The authors noted that the study has limitations due to the study participants came from 1 hospital and most of the responses were positive about the change. The study findings were stated clearly and concluded how nurses positively viewed the change and implementation of bedside reporting in sense of improving patient safety and enhancing their accountability.

The research article covers the importance of bedside reporting in clinical practice. The study highlighted both nurses and patient positive outcome about the change to bedside reporting. The information in the article is about the study conducted was enough information that cover my proposed intervention. The interview consists of two data set (1) clarifying information and intercepting errors and (2) visualizing patients and prioritizing care. Both themes of the study reflect positive outcome and provides opportunity for nurses to make change based on the safety benefits that promotes wellbeing of the patients. In addition, the results of the study prove that implementing this practice to my organization will improve patient flow and allow immediate identification, intercept and correct medical errors. Being an ED nurse, I can take advantage of this practice since I am working in a fast- paced environment and time is vital. I must know my patients in short time and by doing bedside reporting, I will be able to assess and triage my patients accordingly. Authors recommended the need to continue to invest in building organization and the importance to learn effective strategies for transferring information regarding patient care during handoff.

“Bedside Shift Report Improves Patient Safety and Nurse Accountability” is an Article that was written on behalf of Emergency Nurses Association. An article that was based in EBP about the importance of Bedside reporting in the Emergency Department. The quality of the article is good because it discussed the benefits to the staff and patients. Lastly, the authors of the article provided information about how to implement bedside reporting and tips for success in the practice.

The article provided all the information about my project problem and proposed intervention and outcome. According to the article “Patient safety” is the main reason why we strive to implement bedside reporting especially in the ED. It is a fact that ED is a high-risk environment in which safety factors must be considered. Factors included: multiple nurses care for a single patient, high acuity patients with rapid decision of care with time constraints, high volume of patient flow with unpredictable illness and communication barriers to patient and families. With all these factors, bedside reporting can alleviate the stressful environment in the ED because it’s an excellent way to build teamwork, ownership and accountability. In addition, the article also included in respond directly to The Joint Commission’s National Patient Safety Goals including goal #1 :“Improve the accuracy of Patient identification” because reporting at bedside, I will able to meet the patient and checking the armband for identification, goal 2E “ Improve effectiveness of communication among caregivers managing hand- off communication” in which it gives opportunity to ask questions and clarification and lastly goal # 13 “Encourage patients’ active involvement in their own care as a safety strategy” in which patients can give additional information and opportunity to update about their plan of care. These safety goals correspond to intervention and desired outcome. Moreover, the article provided great tips on how to implement bedside report in the ED such as reviewing patients chart before entering the room, ability to perform safety checks such as fall risks, medications, diagnostics/ test performed while updating the whiteboard, and lastly can educate the patient about the latest update of their treatment plan. Being an ED nurse, this article is a great source and provided abundant information that supports my intervention and outcome.

Research: Peer-reviewed article

The quality of evidence in regards to my project is High. The article provided enough evidence as to the cause of the problem, challenges and benefits and provided research study to address the issue. The authors provided sample surveys for Obstetrics unit measuring nurse job satisfaction. It consists of 10 questions to improve shift-to-shift communications between nurses, health team members and patients on the separate units (labor and delivery, nursery, and postpartum). The article also provided a Sample hand-off report sheet for Labor and Delivery and sample protocol for hand-off bedside report on an obstetrics. In conclusion, the article support my intervention that bedside reporting is an issue and must able to adopt this practice to promote safety standards.

The information in the article is relevant to the problem since it provided many facts on bed side reporting. It starts out with background information to key research findings to challenges nurses face today while conducting bedside reporting. Validation of accurate information between shifts promotes patient safety. Another benefit of bedside reporting is the nurses’ ability to streamline their work and avoid unnecessary overtime. The article points out several factors with the implementation of bedside reporting- formulate a committee that include nurses and other team members. Second, develop a measurable goal and a clear plan. Third, a tool must be developed for use during end shift report and lastly, educate the nurses and implement the process in the unit. In addition, the authors discussed that appropriate protocols must be established during the transition using patient-centered care is necessary. Article recommendation supports bedside reporting as best practice standard however further investigations need to be done especially the challenges which includes potential violation of HIPAA.
